Follow-Up Program

The Follow-Up Program at Oasis of Hope Hospital will support you in between your visits for follow-up during and after your initial treatment. It has been our experience that patients who return periodically tend to recover faster and have better long lasting results. Follow-up visits are the most effective method for us to make any modifications to your home care treatment plan and for you to clarify any part of the treatment with your doctor.

From the beginning of your treatment, the follow-up coordinator will meet with you and familiarize you with the home care treatment program and the many ways it benefits you. The follow-up department staff will continue to assist you during your entire treatment and will proactively stay in contact with you for 5 years.

The follow-up department will assist you in monitoring any side effects that you might experience due to your treatment and offer any advice on how to manage them, explain your treatment and any new treatment offered, clarify any questions you may have regarding the effects and dosage of your medication and supplements, and most importantly schedule medical phone consultations with your attending physician.

Before your departure, the follow-up department will schedule a date for your initial medical phone consultation with your attending physician and provide you with a Discharge Check List where you can confirm:

• Date of your next appointment
  
• Special recommendations
  
• Tests to be performed during your home treatment
  
• Prescriptions
  
• Instructions for your at-home primary caregiver
  

Your health is important to us and to circumvent any financial difficulty, the Follow-Up Program is offerred at no additional cost. The Follow-Up Program consists of ten visits to Oasis of Hope over a 5 year period; approximately one every six months after your initial inpatient program. Each visit includes:

• One consultation with your Oasis of Hope physician & a review of the patients recent medical history
  
• A one night stay at Oasis of Hope for the patient and companion
  
• Three meals for the patient and companion
  
• Free round-trip shuttle from the San Diego airport to Oasis of Hope for the patient and companion
