At Oasis of Hope, the Home Care Program equips patients with personalized plans and ongoing guidance to help sustain the benefits of their treatment and support long-term well-being.

Your bridge from the cancer center to home—support between cycles and after completion.

The Oasis of Hope Home Care Program is designed specifically for our patients after discharge from the cancer center—to guide you between treatment cycles or once you’ve completed your on-site program. It brings the same integrative, whole-person approach into your daily routine, so you can keep healing at home with clear steps, regular check-ins, and a team that knows your case.

Who it’s for

  • Recently discharged Oasis patients
  • Oasis patients that are in between treatment cycles.
  • Patients who have finished an on-site program and need medical monitoring at home.

What the program includes

Physician-directed transition plan

Before discharge, your Oasis team creates a written Home Care roadmap based on your diagnosis, recent treatments, medications, labs, and goals. It outlines daily routines and when to contact us.

Follow up care

You’ll have scheduled phone calls to track energy, appetite, sleep, bowel patterns, pain, and treatment tolerance. We adjust the plan promptly when something changes. You will have access to our follow up medical team and scheduled call with our follow up doctors.

Personalized nutrition

Our nutritionist fine-tunes your anti-inflammatory, protein-forward meal plan for home life. You’ll receive shopping lists, simple recipes, batch-cooking tips, and strategies for common issues.

Targeted supplements & liposomal supports

Based on your case and labs, we recommend a streamlined supplement plan—prioritizing quality, timing, and compatibility. Liposomal options are recommended to support absorption when digestion is sensitive. Your home care liposomal nanoceuticals are included in your treatment protocol.

How it Works

  • Discharge planning – Before you leave the cancer center, we finalize your Home Care roadmap and schedule your first follow-up.
  • Weekly rhythm – Regular check-ins (more frequent if needed) keep you on track; we update your plan as you regain strength or start a new cycle.
  • Lab monitoring – When appropriate, we order labs locally and adjust nutrition, minerals, and supplements based on results.
  • Escalation & return-to-center – If you need more intensive support, we’ll help you return to Oasis of Hope Cancer Center.

How it supports treatment cycles

  • Between cycles: We focus on recovery and readiness—nutrition, minerals/electrolytes, rest, gentle movement, and symptom relief—so you’re prepared for the next treatment block.
  • After completion: We shift toward monitoring—surveillance of key symptoms and labs.

Safety First

Your Home Care plan is clinician-directed and personalized. You’ll receive clear instructions for urgent symptoms and 24/7 guidance on when to seek local emergency care. We adjust or stop elements that aren’t helping and keep communication transparent at every step.
