What is Tumor Infiltrating Lymphocyte (TIL) Therapy?

Tumor Infiltrating Lymphocyte (TIL) Therapy is a personalized immunotherapy that uses the patient’s own immune cells, harvested directly from within their tumor, to fight cancer. TILs are specialized lymphocytes that have already recognized cancer cells and migrated into the tumor microenvironment to attack them. However, inside the tumor, these lymphocytes often become “exhausted” due to immune suppression and the harsh conditions created by cancer. At Oasis of Hope Cancer Center, TIL therapy revitalizes and expands these naturally tumor-specific cells outside the body, then reinfuses them in large numbers to provide a powerful, targeted immune response.

How the Therapy is Developed

The process begins with the collection of tumor tissue, either through surgery or biopsy. From this tissue, lymphocytes that have already infiltrated the tumor are carefully isolated. In our cGMP-compliant, ISO-certified laboratory, these TILs are cultured with high-dose interleukin-2 (IL-2) and other immune-stimulating factors to restore their activity and promote large-scale expansion. Over several days to weeks, billions of highly active, tumor-specific lymphocytes are generated, producing a robust supply of immune cells that are uniquely trained to recognize the patient’s cancer.

How it Works in the Body

After expansion, the TILs are reinfused intravenously into the patient. Because these lymphocytes were originally harvested from within the tumor, they carry the ability to recognize cancer-specific antigens with precision. Once reintroduced, they circulate throughout the body and return to tumor sites, where they directly attack malignant cells through perforin- and granzyme-mediated apoptosis. Unlike other therapies that require broad stimulation, TILs provide a highly targeted response, focusing their cytotoxic activity on cells that express the exact antigens found in the patient’s tumor. This precision increases treatment effectiveness while minimizing damage to healthy tissue.

Advantages of TIL Therapy

TIL Therapy is uniquely powerful because it amplifies the patient’s own natural anti-tumor response. Unlike therapies that rely on predicting or engineering tumor targets, TILs are derived from cells that have already demonstrated the ability to recognize the patient’s cancer. This makes them highly specific and adaptive to the tumor’s unique antigen profile. Furthermore, TIL therapy creates a population of memory T cells that continue to patrol the body after treatment, helping to provide long-term immune surveillance and reducing the risk of recurrence.

TILs in Combination Therapies
